Recommended Well Child Visit Schedule  

In addition to having your child immunized, it is recommended that your child have periodic well child visits, which are generally done at the time of when immunizations are given. The following schedule is a general recommendation. Please consult your child’s physician for an individualized schedule.

Birth

Physical Exam
includes height, weight, head circumference measurement, urine stream, congenital disorder screen, eye prophylaxis, newborn hearing screening .

Counseling
may include child safely related to seat belts, smoke free environment, measures to decrease the risk of unintentional injuries, encourage back sleeping position, check on fluoride percentage in the local water supply, counsel on the danger of driving under the influence of alcohol or drugs

1 Month

Subjective vision and hearing

Galactosemia screen – a blood test which detects an intolerance to lactose/milk products

Thyroid screen

PKU Screen

2 Months

Physical Exam
includes height, weight, head circumference, subjective vision and hearing

Developmental milestones
lifts head and chest, makes sounds follows object across midline

Counseling
may include child safely related to seat belts, smoke free environment, measures to decrease the risk of unintentional injuries, encourage back sleeping position, check on fluoride percentage in the local water supply counsel on the danger of driving under the influence of alcohol or drugs

4 Months

Physical Exam

grasping at objects, follows objects with eyes, turns had toward sounds

6 Months

12 Months

Physical Exam
includes height, weight, head circumference, subjective vision and hearing

Developmental milestones
sitting alone, pulling up to walking, follows simple commands and copies others, assessment of language development

TB Screening for high risk children

Hemoglobin/hematocrit for high risk children

Lead screening – for high risk children before 15 months

Counseling
may include child safely related to seat belts, smoke free environment, measures to decrease the risk of unintentional injuries: child proof toilets, electrical outlets and cords, cleaning materials, etc. Counsel to promote health diet, transfer to whole milk. Counsel on the danger of driving under the influence of alcohol or drugs

15 Months

Physical Exam
includes height, weight, head circumference, subjective vision and hearing

Developmental milestones
walking alone, give and takes toys, drinks from a cup held in both hands, listens to stories and points to pictures, assessment of language development

Hemoglobin/hematocrit for high risk children before 15 months

2 Years

runs, jumps and throws a balls, scribble with crayon/tool, assessment of language development – generally says up to 50 words, uses 2-3 word sentences

3 Years

assessment of social and language skills

4 – 8 Years

During this period, this child should continue to have yearly well visits to his/her provider.

Physical Exam
includes height, weight, blood pressure, vision and hearing screening

Developmental milestones
assessment of social, speech and language skills

Lead screening – for high risk children

PPD, high risk and offer Mantoux

Counseling
inquire about school performance, appropriate and inappropriate touch, decrease the risk of unintentional injuries, seat belt safety, bicycle safety with helmets, elbow/knee pads, promote physical activity, maintain smoke free environment, counsel on danger of operating a motor vehicle under the influence of drugs or alcohol, counsel on routine dental visits, and promote and healthy diet.

9-12 Years

During this period, this child should continue to have well visits with his/her provider every 2 years.

Physical Exam
Blood pressure, height, weight, assess physical and social development, objective vision and hearing

Hematocrit/hemoglobin – for high risk children

Counseling
inquire about school performance, appropriate and inappropriate touch, decrease the risk of unintentional injuries, seat belt safety, bicycle safety with helmets, elbow/knee pads, promote physical activity, maintain smoke free environment, counsel on danger of operating a motor vehicle under the influence of drugs or alcohol, counsel on routine dental visits, promote and healthy diet, and routine dental visits. Counsel on sexual maturation, encouraging abstinence, puberty and development. Counsel about sexually transmitted diseases.

15-18 Years

During this period, this child should continue to have well visits with his/her provider every 2 years.

Physical Exam
blood pressure, height, weight

Rubella screening for females of child bearing years

PPD, high risk and offer Mantoux for 11-16 year olds

Counseling
Inquire about school performance. Counsel on sexual maturation, encouraging abstinence, puberty and development. Counsel about sexually transmitted diseases. Take measures to decrease the risk of unintentional injuries, seat belt safety, maintain smoke free environment, and promote physical activity, routine dental visits, healthy diet and counsel against illegal drug use. Counsel on the danger of operating motor vehicles under the influence of alcohol and other drugs.
