Network. If you are a health care professional
who would like to apply for network participation,
the approved applications are below in PDF format

Upon receipt of your request, you will receive a packet
that includes the following:
- Contract
- Sample fee schedule
Additionally you may want to review the following information,
available online:
- Check
List (identifies what documents are required prior to
contracting)
If you are practicing in one of the following states that
requires a specific application, you can link directly to
the state-specific application below.
Please remember that sending in an application does not
automatically make you a network participant. A First Health
associate will send you a contract and rate information. Additionally,
there are a number of items that need to accompany the application
you submit, so be sure to review the check
list to facilitate the contracting process.
