First Health offers a special program to help curb unnecessary costs for medical conditions. Disease management concentrates on many of the highest-cost conditions:

	Chronic Obstructive Pulmonary Disease (COPD)		Diabetes
	Congestive Heart Failure		Chronic Renal Failure
	Specialized Wound Care		Transplant
	High Risk Maternity		Depression

Because we maintain all medical and pharmacy data in a single database, we are able to immediately identify and start helping policyholders who would benefit from this service. We offer coordination by a single nurse case manager for policyholders with multiple chronic conditions, to support consistency of care. Nurse case managers are focused on all aspects of policyholders' care.*
