First Health endorses use of the Council for Affordable Quality HealthCare (CAQH) online application process for practitioners. The Universal Credentialing DataSource (UCD) is a secure online credentialing application database that is free to practitioners and can be released to any of your affiliated organizations or hospitals who will accept this form. It has been mapped to provide mandated state application where applicable. This process is supported by the AMA, AAFP, ACP, MGMA et al.
